Vendor Registration

This is not an electronic registration form. It can be used in one of two ways.

  1. Fill in the form with all of your information and then print it out. Then mail with your deposit to the address listed below.
  2. Print out the form and write in the information. Then mail with your deposit to the address listed below.

There are 3 ways to be involved

1. Yes, we would like to be at the site to have our display up, to sell our products, and to meet the coaches. Enclosed is a $125.00 check, we will bring the door prizes with us.
 
Make check payable to MIDWEST VOLLEYBALL INSTRUCTORS. Please send reservation form and check before March 13.
 
Directions and hotel accommodations can be found on the web site. Shipping information is below.
 
2. Yes, we would like our advertising pieces in the clinic vendor packet. Enclosed is a $60.00 check made out to MIDWEST VOLLEYBALL INSTRUCTORS. We will send 200 advertising pieces and the door prize(s) to:

Please send stuffing pieces, door prizes and check so that material arrives before March 10.
 
Any material that arrives after March 10th we will put out on tables and encourage coaches to take, but we can not guarantee that each coach will receive one.
 
3. Yes, we would like to advertise in the 80+ page, GBC bound MVI CLINIC NOTEBOOK which contains each clinician's notes and diagrams which each participant receives.

___ The outside back cover(Cover 4) sells for $ 75.00.
___ The inside back cover(Cover 3) sells for $ 60.00.
___ A part of the ADVERTISING SECTION at the end of the Clinic Notebook

To participate in the CLINIC NOTEBOOK- Deadline for artwork - March 1.
 
1) Page size is 8.5 x 11
2) Camera ready artwork only - black and white only
3) Booklet will be photocopied
